Understanding the Register Format

Cover Sheet

Section 2: Access Conditions

Section 3: Authorised Viewers
Names, contact details, and scope of access for individuals permitted to read some or all of the register.
Audit Trail Volume
Maintained as a separate append-bound book or sequenced addendum. Each entry receives a number, date, verifier signature, site code, and action description. Retroactive entries are flagged when reconstructed from prior records.
